I'm not sure why you created a new thread.. you could've just edited the first one?

You seem to be trying to include everything the template asks for, but you need to actually use the template itself, copy it and paste it into here and put your answers next to each line, that is our protocol because it makes it much easier for the ban appeal process if we can clearly see that you've addressed each point in the template.
